A relationship can be defined as mutual exchange of emotions between any two individuals with a long term commitment to each other.
This can pre or post -marital, while married or in the face of a separation or Divorce. The other common relationships are Parent- Child and Siblings.

Some common signs that the relationship is having issues or is getting worse-

When both partners or individuals start having trouble expressing themselves, or one partner reduces general conversation , even stops conversing.

When no agreement happens on in any discussion, and always leads to a big fight.

When any one or both start hiding things or even start lying.

Distorted or no decision ever gets made with mutual consent.
I follow the method of first listening to each individual separately. This gives me an unbiased perspective of both the parties involved. The next step is to have a combined question- answer session highlighting the core issues being faced by both the individuals. This question- answer session reveals the trigger points for both. I then give a series of situations to be enacted and practiced by both where both the parties have to get over their individual blocks. Common interests and mutual respect for each other personalities is also explained.
